
WHAT IS A NEXUS MEDICAL RECORD REVIEW
A Nexus Medical Record Review is a structured medical evaluation designed to assess whether a connection between a veteran’s diagnosed condition and military service may be medically supported. This review is required before a medical nexus letter can be considered and ensures that opinions are based on documented evidence rather than assumptions or outcomes.
This process focuses on medical analysis only and does not involve legal advice, VA claim filing, or representation.
WHY IS A MEDICAL RECORD REVIEW
REQUIRED BEFORE A NEXUS LETTER
Medical nexus opinions must be grounded in objective medical evidence. A medical record review ensures that all relevant records are reviewed and that a medical provider can determine whether a nexus opinion meets accepted medical standards and VA evidentiary requirements. Not every case supports a medical nexus opinion. The record review process protects both the veteran and the medical provider by ensuring opinions are issued only when medically appropriate. The record review process helps determine:

Whether a current diagnosis is documented

Whether there is evidence of an in-service event, injury, illness, or exposure

Whether medical records support continuity or progression of symptoms
-
Whether a nexus opinion can be medically supported


WHAT RECORDS ARE REVIEWED DURING MEDICAL REVIEW?
The medical provider evaluates available documentation relevant to the claimed condition. Records may include, but are not limited to:

Service treatment records

Post-service medical records
-
VA medical records
-
Private medical records
-
Diagnostic reports and imaging
-
Prior VA decisions or examinations (if provided)
Veterans may upload additional records if new or relevant documentation is identified during the review process.
POSSIBLE OUTCOMES OF THE MEDICAL REVIEW
After reviewing the records and consultation information, one of the following outcomes may occur:

Nexus Opinion May Be Supported
If the medical evidence supports a connection consistent with accepted medical reasoning and VA standards, a medical nexus letter may be prepared. -
Additional Records Needed
In some cases, additional documentation may be required before a determination can be made. -
Nexus Opinion Not Medically Supported
If the evidence does not support a medical nexus, no nexus letter will be issued.
A nexus letter is never guaranteed. Medical opinions are provided only when supported by evidence and professional judgment.

HOW THE MEDICAL REVIEW LEADS TO NEXUS LETTER SERVICES
When the medical review determines that a nexus opinion may be medically supported, veterans may proceed with applicable nexus letter services. These services are offered separately and depend on the number of diagnoses and complexity of the case.
Additional services, such as addendums or updates, may be available if new medical evidence is introduced after the initial review.
